1. On the DS-156 form theres question 2 It asks for the City, Country, State of Issuance for my passport. Would this be the city i live/was born in. Or would it be London (since I'm a UK citizen

2. Do I send in the photos for the form now? Or wait until the interview?

3. Do I sign the declaration part Q.41 now? I know that you don't sign the part on the DS-156K form

4. For the check list that i fill in for step four, What Do I do for parts that don't apply to me e.g Adoption Certificate, and the section for children?

Dec. 5, 2005 - Sent K1 application to TSC (Day 1)

Dec. 7, 2005 - K1 application signed for & delivered to TSC (Day 3)

Dec. 14. 2005 - NOA1 from CSC (Day 10)

Feb. 28, 2006 - email notification: Approved NOA2 from USCIS!!

Mar. 20, 2006 - written notification application package is on it's way to London embassy!!

Mar. 31, 2006 - written notification from London embassy with instructions/forms/etc. to send back.

I put Newport as that is where my passport was issued. If it was issued in London then yes put that.

2. Do I send in the photos for the form now? Or wait until the interview?

No, they go with you to the interview.

3. Do I sign the declaration part Q.41 now? I know that you don't sign the part on the DS-156K form

Yes if you are ready to send back the forms.

4. For the check list that i fill in for step four, What Do I do for parts that don't apply to me e.g Adoption Certificate, and the section for children?

Only mark the ones that apply to you.

27 Feb. 2006: K1 petition sent to Nebraska Service Centre

2 March 2006: NOA1 dated (received in mail on 9 March 2006)

4 May 2006: NOA2!!!!! (DAY 66)

8 May 2006: File Sent to NVC (as stated on email dated 9 May)

24 May 2006: NVC sends our file to London Embassy

8 June 2006: Packet 3 arrives (DAY 101)

25 August 2006: Pack 3 sent to London Embassy registered post

29 August 2006: London Embassy receives my Pack 3 - 9/9/06 pack 4 arrives

SEPT 2006:APPLICATION PUT ON HOLD - WE HAD A FALLING OUT.

MAY 2007:RESUMED THE VISA APPLICATION

6 June 2007: Medical @ 2.30pm - need to get anew GP letter and psychiatric evaluation

7 June 2007: INTERVIEW @ 10am - waiting for new police record letter

3 July 2007: APPROVED/Visa arrives

I can't find on my passport where it says it was issued :blush:

Dec. 5, 2005 - Sent K1 application to TSC (Day 1)

Dec. 7, 2005 - K1 application signed for & delivered to TSC (Day 3)

Dec. 14. 2005 - NOA1 from CSC (Day 10)

Feb. 28, 2006 - email notification: Approved NOA2 from USCIS!!

Mar. 20, 2006 - written notification application package is on it's way to London embassy!!

Mar. 31, 2006 - written notification from London embassy with instructions/forms/etc. to send back.

But when we turn to the Hebrew literature, we do not find such jokes about the donkey. Rather the animal is known for its strength and its loyalty to its master (Genesis 49:14; Numbers 22:30).

lol no just change of circumstances delayed us sending it off

ok there are 7 regional offices but there is a phone number you can call to get the information you require....
