Nonexistant forms?

My fiancee's congressman's office just told us we need to fill in a form IV15. This form, as far as I can tell, does not exist. Does it? And if not, what should I do?

If my memory serves me right (which at times it dosent)...Its the Consulate/Embassy checklist!! try getting it from your Embassy's website!!

Ummm..ok, I'm not sure on this, but IV15 is the checklist sent back with Packet 3 that has to be returned to the Embassy (for London, I don't know if this applies to other embassies). What Embassy are you going through and are you even at the Embassy stage yet?

I don't think I can help you much more than that because you didn't mention where you are in the process right now. Did you send the K-1 yet? Did it get approved?

Well, we sent that off as soon as we got it. Now the embassy are saying they don't have it.

Well, we sent that off as soon as we got it. Now the embassy are saying they don't have it.

AH i see well this depends on how long ago you sent it??........as the Embassy some times says they didnt get it yet if its only been a couple of weeks , as soon as the manualy process your papers & input them into the computer will they say they have papers.........so maybe try emailing them to ask if they have got them yet!!

Which Embassy is it?

IV-15 is the old name of the DS-2000 which is, indeed, the checklist.

Well, we sent that off as soon as we got it. Now the embassy are saying they don't have it.

Then probably you'll have to send it again Contact the embassy and ask them to send you the form again, or maybe you can download it from their website. Good luck!

Or maybe it's stuck somewhere in their mail and they didn't receive it yet.

We sent it months ago. And, they can't be bothered to embed the required fonts into their forms? Cause I can't view that IV15 without a 300 euro font pack.
