I want to go ahead and get this form filled out that way when it's needed, it will be done. But the ones I am finding all say it expires 07-31-2007. Does anyone have a link to an updated one?

Thanks!

Tammy

It is the most current form, they haven't released another one - so it is ok to use. Just make sure, when the time comes to send it off, that they haven't replaced it with a newer one.
