Thanks, that's a long way from Virginia, or are you saying that the doctor would fill-in I-693A from DS-3025 by mail?

He certainly will! He's a long way from Tennessee, too, but I just phoned, explained my situation. They told me what I needed to send and off we went! :)

I got my TD & MMR done before i went for my medical in London as the Local Health Authority shredded all immunization records when u reach the age of 25 yrs. So i had them done Uk side, the Panel Physician noted them on the DS-3025 which i had transcribed over to the I-693A by my local CS for FREE!! he must have been in a good mood that day :lol:

Saved me $$$ Stateside anyways!!

I had all my vaccinations complete on the DS 3025 and at my interview got a RFE for the I693A. I sent mine off to Dr Arnold and returned that to USCIS.

Atlanta (the office I had) is a pretty strict office as they are also requesting alot of applicants to have the TB skin test by the look of things lately.

I hadn't heard until now that there was a second TB test that we 'might' have to take....... has there been many VJ'er's asked to provide the skin test results?

There were some threads going around about this a few weeks back. Seemed it was isolated to a few local offices - but no one could find evidence that its any sort of 'requirement'.

At this point, the most I would do is look on uscis.gov at the civil surgeon locater to find the closest docs. If one is handy, you might give them a ring to discover their procedure for completing the form should you need it.

I wouldn't spin my wheels until I got an RFE - if I knew I could resolve the problem easily and quickly.

Easily found a civil surgeon just around the corner.....

Just phoned a local ocivil surgeon up and they knew what I was referring to right away (good sign)...... and I could have the I-693A completed whenever for the mere price of $278........ not cheap, but good call - at least I know I could react to the RFE quickly if needs be.

Good grief! He MUST be charging you the full medical price for that, there is no way in hell they can substantiate $278 for some simple copying of information from one form to another.

A decent price is somewhere in the region of $30, so shop around or go with Tracy's suggestion of the CS in Texas.
