Dear Sir or Madam:

Enclosed please find the following forms and supporting documents:

Forms:

I-485, Application to Register Permanent Residence or Adjust Status

G-325A, Biographic Information

I-864, Affidavit of Support Under Section 213A of the Act

I-765, Application for Employment Authorization

I-693A, Vaccination Supplemental Form

Supporting Documents:

1. Copy of Passport

2. Copy of K-1 Visa from Passport

3. Copy of Notice of Approval 2 for I-129F

4. Copy of valid I-94 (front and back)

5. Copy of birth certificate in English.

6. Certified copy of marriage certificate.

7. Two passport style photos. (for I-485 and I-765)

8. Copy of form W-2 and 1099 (most recent)

9. Copy of most recent tax return 2006.

10. Copy of pay stubs from the last 3 recent paychecks.

Copies of documents submitted are exact photocopies of unaltered original documents, and I understand that I may be required to submit original documents to an Immigration officer at a later date.

Your consideration of this matter is greatly appreciated.

A cover letter isn't necessary, however if you are going to make a checklist like this, its useful to include everything you need to include in the package for your own uses. So for this reason I say: What about the check?

Also any reason why you're not applying for AP? Its free, so you might as well.

I would recommend Cover Letter, there is an example on this Forum. Yours looks pretty close

I would recommend you include:

the check amount for the fee and biometrics

your alien number

your SSN

I added a photocopy of my EAD card

some will say the AOS Cover Letter is unnecessary. I found out the hard way it probably isn't necessary. it will be separated from the application along with any other unnecessary documents.
