vaccination & affidavit question

Hi all, my wife and I are just about ready to file for AoS and i have 2 minor questions. When she arrived at the Atlanta Airport on her K1, the immigration officers took her sealed envelope from the civil surgeon. She did get to keep what appears to be a vaccination supplement but it's slightly different and doesn't specifically say anything about I-693. Will it suffice? It's got a stamp on it but i'm guessing a copy will suffice as well.

The 2nd question is a real noob question. The very first question on the I-864 says either i am (a) the petitioner of an immigrant, or (B) i filed an alien worker petition on behalf of a relative. I'm guessing (a)?

1) Civil surgeons are only stateside. Amongst many things in the sealed envelope from the consulate was the medical report performed by a panel physician. The vaccination report you have in your possesion can be transcribed by a civil surgeon to the form commonly referred to as the I-693A. Prices for this service vary greatly by provider so call around.

The civil surgeon in Kyiv was approved by the US, he was the only one. The form i have is completely in English except for the stamp from the surgeon which is in Ukrainian. Do i still need to find a civil surgeon over here to redo the form before i file for AoS?

The surgeon in Kyiv was specifically for the visa; the certification you need now is for adjustment of status here in the states, hence you need a US civil surgeon.

Medical was performed by an approved panel physician, not a civil surgeon (not important distinction, but still different). Anyways, some people have had success submitting only the vaccination record received from abroad (what you have) in lieu of the I-693A. Your call......

The instructions of the I 485 state that the medical for a K1 is for the non immigrant visa, and it only needs to be redone for AOS purposes if that medical was over a year ago. In that event, I would also assume that you'd want to do it here - not overseas - same for the I 693A. But I guess nothing would stop you from flying back over and having the I 693A done there.

I assumed that if an overseas medical doctor filled out the I 693A, it would not be accepted for AOS. Or would it be? Do you know?

I went ahead and called USCIS and amazingly i didn't have to wait at all. The woman told me to just send the original document we received in Kyiv and that will be just fine.

On the Visa it says Issue Date 22May2007 and Expiration Date 20Nov2007, but the immigration officer marked on it and drew a line to the next page which is stamped and says Sept, 03, 2007. I'm assuming 9/3/07 is what i should put on my I-485. She arrived on May 5th so i'm not sure where he got 03 from.

90 days is the length of approved stay, not 3 months. so September 3rd is correct.
