everything needed for AOS?

i'm finally going to be sending in all my paperwork for my AOS, EAD and AP and i just want to make sure i have everything i need. this is what's on my coversheet:

Enclosed please find copies of required forms and supporting documentation as indicated below.

-Payment in the amount of $1010.00

-I-485 Application to Register Permanent Residence or Adjust Status, plus supporting documentation including:

-2 passport-style photos, current

-copy of birth certificate

-copies of K1 approval notice, visa and I-94

-copy of marriage license

-G-325A Biographic Information

-I-864 Affadavit of Support, plus supporting documentation including:

-most recent 3 years of tax returns/W2’s

-evidence of employment (paystubs)

-I-765 Application for Employment Authorization, plus supporting documentation including:

-2 passport-style photos, current

-copy of birth certificate

-copy of I-94, K1 Visa and passport ID page

-copy of Social Security card

-I-131 Application for Travel, plus supporting documentation including:

-2 passport-style photos, current

-explanation warranting issuance of I-131, completed as per Part 7

-copy of I-94, K1 Visa and passport ID page

-PLEASE NOTE: Vaccination information was included in the envelope with my medical results; this was surrendered to the POE officials, to be forwarded to USCIS, when I crossed the border at Pembina, North Dakota on June 28, 2007.

so, do i have everything needed? if not, what exactly am i missing?

if i do, what do i have to do? they have the results of my medical and vaccinations... i do have copies of proof of vaccinations so would i have to take that to the doctor in order to get the I-693 (i think that's the document number)?

The vaccination supplement is the I-693A. You could take your records to the doctor, and he'll copy them onto the form and sign and seal it.

if i get an RFE for this, i'll get it then - no sense spending money if i don't have to... or is it better to do this ahead of time and send it in regardless? i don't want to hold things up, but money is also tight right now with me not working and my EI not coming in yet...

From our guides:

I-693, Medical Examination of Aliens Seeking Adjustment of Status. (If you already have this from your overseas Visa Interview submit a copy that medical check form; No fee is required in this case. If you are a K-1 fiancé(e) or K-2 dependent who had a medical examination within the past year as required for the nonimmigrant fiancé(e) visa, you only need to submit a vaccination supplement (I-693A), not the entire medical report. You may include the vaccination supplement with your adjustment of status application.)

Do what you want but check this out. You are setting yourself up for a RFE.

No biggy I guess but when they find one easy thing to RFE you for they usually check more closing and you end up with a couple more BS ones.

FEDERAL REGISTER INTERIM REGULATIONS at USCIS website, I quote:

"C. Medical Examinations

According to 8 CFR 245.5, aliens seeking to adjust status are required to undergo a medical examination performed by a designated civil surgeon to determine whether they are inadmissible under section 212(a)(1)(A) of the Act. To date, applicants for K nonimmigrant visas have been required to obtain a medical examination abroad pursuant to Department of State regulations at 22 CFR 41.81 prior to entry, and the medical examination is not repeated if they apply for adjustment of status within 1 year of the dat e the examination was performed. They are, however, required to submit with the adjustment of status application a vaccination assessment completed by a designated civil surgeon in order to establish their compliance with the vaccination requirements under section 212(a)(1)(A)(ii) of the Act.

The Service will continue this same policy for the K-3/K-4 nonimmigrants. K-3/K-4 nonimmigrants who file their adjustment of status application within 1 year from the date of the medical examination overseas will not have to submit an additional medical examination. However, the Service notes that applicants whose medical examinations overseas revealed a “Class A” or “Class B” condition (as defined by 42 CFR 34.2( B ) ) must establish upon application for adjustment of status that they complied with those cond itions imposed on the initial admission. Failure to comply with those conditions means that a new medical examination will be required.

Please note the bolded section.

Many members here have brought it along to their AOS interviews however, if you get a CSC transfer, you will probably not get interviewed and therefore will be lacking in the vaccination supplement area.

They will RFE you for it and it WILL result in a delay.

It is up to you, of course, whether you are willing to take the risk on not including it in your AOS package, because you are going to need the supplement at some point, whichever way you look at it.

*sigh* then i guess i should get it done... however i don't want to delay sending in my EAD any longer (applying in with AOS). if i were to submit my AOS/EAD/AP application now, then get the vaccination form completed by the doctor and send it in for inclusion with my file, would this work? or would it just make a mess of things at their end?

Someone can correct me if I'm wrong, but there is no way USCIS will be able to match up your vaccination supplement if you send it in after sending in the AOS package. If you don't include it then just wait for the RFE, as thats the only way they'll be able to match it with you're application.

Do not just send them stuff. It will probably get lost. Only do so if requested.
