You can do CFO in Cebu or Manila. May as well do it and get it out of the way as completion of CFO is also required for immigrants leaving the Philippines.

Also you need to do the seminar part for your changing name to surname on passport.

Per Republic Act 8239 and Department of Foreign Affairs (DFA) Order 28-94, Filipinos proceeding abroad as fiancé(e)s and spouses of foreign nationals are required to attend the CFO's guidance and counseling programs as a pre-requisite to the issuance of passports.

For renewal of passport:

  1. Three (3) copies 4.5 cm x 3.5 cm new photos (colored with plain white background). Photo should show applicants in decent attire with collar.
  2. Present old passport and photocopy of passport pages 1,2,3 (amendment) inside and back cover. The pages showing latest Bureau of Immigration departure and arrival stamps.
  3. For married women who chose to adopt surname of husband in her new passport, Marriage Contract in Security Paper issued by the National Statistics Office or Certified True Copy and photocopy of MC issued by NSO.
  4. And since you're married to a foreigner, the CFO Guidance and Counseling Seminar Certificate (Yellow Certificate).

You can attend the CFO Seminar in Manila or Cebu (whichever is closer to you). You will also be needing the Yellow Certificate onced you get your visa for the CFO stamp (on the passport) and the green certificate-> this is a requirement before you will be allowed to leave the Country.
