They should give you a little vaccinations booklet when you get the jab. Have you already had the Tdap as well? If so, you'll need a printout from your GP to show that this has already been done.

When i had my medical they jus said i needed the MMR and HPV but seems i dont need the HPV now but i still need the MMR and i can get it done by my doctor but i know that when i AOS ill need to prove iv had it and i was wondering how i go about it.

You might ask the doctor for a copy of the DS-3025 (which is the form they fill out for the embassy) when you have your medical. It can make things easier when you file for your adjustment of status. My fiance asked for one and the doctor made sure to provide a copy for free. :) Good luck!

They told me that they would post the form to me (after they received some additional vaccination info from the GP) and I didn't even ask for it. Even got a "good luck" and smiley face on the compliment slip lol

I hope this isn't a very repetitive question, but after searching through the many 30 something pages of this thread, I just want to make sure that I have it clear because I'm making myself confused.

My fiance has his medical appointment next month, but he had all of his vaccinations done through a different doctor in the UK. Is there a specific form for the medical that he needs to have filled out in advance by the doctor to prove he is up to date with his vaccinations or can the doctor just type up a letter? What is the best way to go about this? It is very different from how we get our records in the U.S. and my fiance isn't sure himself what he should request for proof as well.

Hi Michelle,

My fiance did the same thing--got his vaccinations done through his local GP. He asked them to record the vaccinations down and they did, I don't think they even used a form, just documented what he had and signed it. When I'm able to talk with him later tonight, I'll ask him exactly how they documented it. Good luck to you and your fiance!

Had my medical today at 12:50 ive got to say i was really nervous (for some stupid reason) but it was the easiest thing i have ever had to do!!!

Got the train from Liverpool to Euston: 2 hour journey (AWESOME!)

Arrived in London and then proceeded to get a taxi from Euston station to Bentinick (?) Street: 15 mins? (About £7)

Was early so walked to Oxford street and had a little walk around the shops and walked back to the doctors at about 12:40, pushed the buzzer said who i was an i was in! From the outside it doesn't look like your typical doctors practice, but that is probably because its private and im not used to it! :P

Go to the little window hand in your form, passport, passport photo and vaccination report and then ull b sent to the waiting room. About 5 minutes later my name was called, i got my passport back, filled in a form and then about 5-10 minutes later my name was called.

Everyone was really friendly in there an the nurse that took me in probably knew i was nervous an was laughing an joking with me (which helped!!!) :P

She looked through my vacc records and noticed it was over 10 years since i had a tetanus jab. It was either don't have it there an then an my documents would be held until i got the jab from my doctor OR pay £25, get the jab and my documents would be updated and released that day. I opted for the latter to save any messing but for all you people that are going for your medical in the next month or so, go to your doctor and get it done for FREE (most likely) and get an updated vaccination report. Otherwise be prepared to pay £25 like me :P

After that I went for my chest x-ray about 5 minutes later. Shirt off, stand here, hold ur arms like this, chin on here, deep breath annnnn DONE! I was in there for about a minute tops ha!

Then back to the waiting room and about 10 minutes later an Irish male dr came an got me took me into his room an performed a physical. A few routine medical questions, check weight & height, check ur heart, check your chest, eyes, throat, ears, check your stomach and pelvic, check your blood pressure, take your blood pressure. "Your all done good luck in the states!"

Payed and then left!

It was so easy and so quick. I was in there for about 40 minutes tops lol! :P Everyone is really friendly and efficient.

I hope this eases some peoples minds :)

*edit* they asked if i had ever had chicken pox. i didnt know so they wrote no on the DS-3025 form (which she gave me a copy of without asking!) apparently sometime in the future when i am in the states i will have to have the vaccination. so make sure you find out :P

I went into my doctors surgery and just asked the woman at reception if she could give me a printout of my vaccination record and within a few seconds she did! :P

I am getting all my paperwork together and i have just filled out the medical questionarie, for the check boxes at the bottom of the page the first question is "Have you ever been hospitalised (including phychiatric admission)?".

I was bitten by a police dog and had to go to hospital (i wasnt running away just incase you wondered it was in training and didnt listen to the handler) and i have also been in when i stabbed my hand with a screw dirver, again it was nothing like self harming i slipped when tightening a screw and the screwdriver went traight into my hand, there are a couple of other instances like big cuts and stuff that got me to A&E in the past.

Would these be classed as being "Hospitalised"? and should i answer yes and give details.

Hospitalised I took as meaning if you had an overnight stay for anything. I did back when I was 5 so I declared that.

They happily gave a copy of the DS-3025 to my fiance without him even asking, but I had warned him ahead of time to ask for it in case they didn't automatically give it out! Good luck!
