DS-3025 is an immunization sheet done at the visa medical. You will be given a photocopy but you won't need it for immigration purposes. Your original DS-3025 goes to the embassy with the medical results and will show you are cleared for the spouse visa and greencard. You do not adjust status in the US. You are a permanent resident the day you enter.

 

Your vaccination history:

You are wanting record of MMR (mumps, measles, rubella) which are typically baby shots.

If you have proof of two MMR, then no more needed for visa. 

If you have proof of one MMR, then another due for visa.

If you have no proof of MMR, then get one at the GP and the second will be waived if four weeks have not passed when you go to your medical. They can't give your the second one closer than four weeks so it is waived for "insufficient time interval." IF it is waived, you never have to get it.

 

Next shot needed is Tetanus, Diptheria, Pertussis. It may be on your childhood records as DTP. P is pertussis, not polio. It's a childhood series of three shots you may already have. If so, and your last one was less than 10 years ago then you should be okay. If you don't have records then you may need a Td booster like ChellyandAaron had to get. That's a new twist at Knightsbridge and maybe a new shot person insisting on three previous before they will give you credit for the one you had on 2009. 

 

Varicella shot- if you tell them you had chickenpox, it is waived. No written record needed. But as a CR1, it can also be waived for the reason "not routinely available" because difficult to get in the UK and you must be complete by your interview. So they waive it since you can't get it in the UK, even at Knightsbridge. You never have to get it if waived.

They will contact you within four to five days if there are any issues with the results of any of your tests.  They tell you that if you don't hear from them, that everything is okay.  I had to collect additional information and get my doctor and a hospital to fax and email the information to them.  When I phoned them on the Friday after my medical to make sure that they had received everything, they told me it had all been received the day before (last Thursday) and that my medical was signned off and on its way to the embassy - and that it should all be 'smooth sailing' from here.

OK great. Do they really check your genitals? That seems a little over the top? What are they looking for? Lol!

I didn't have a genital check.  I think they phased this out last year.  I think it was originally to check that you were male/female - since equal marriage laws were passed it probably isn't necessary any more.  I guess if there is a recent STD they might check for something but otherwise you can keep your pants on!

Timeline

The doctor told me it was to check you are the gender you say you are. I asked if he got many that weren't. He said "you'd be surprised."  Agree they seem to have quit that. It was a half-second peek when I went and didn't traumatize me.

My wife had her exam today.

 

<10 minute wait

Gave all the forms to the front, who didn't really look at anything

Taken by nurse initially, got urine done. Doctor was a little strange, questioned why she was given Bactrim in the past (came up because she is allergic) and it's not like my wife could explain why that drug vs. another. No genital exam (He even said "I will not be doing the genital exam today"), had to strip down to bra/pants though. Doctor is the one who drew blood, wasn't very good at it (couldn't find a vein and wasn't successful first time). Then went to X-ray.

 

In the end it cost 302 pounds - 290 for the exam, 12 for flu jab

Sorry for the late reply and thank you.  My medical is tomorrow.  Is the form DS 3025 something that is handed to me after the examination is complete or is it mailed to myself or the embassy?  If it is the latter how do I ensure that I receive the second page that you mentioned?

They routinely give a photocopy to everybody. Your original DS-3025 that goes to the embassy will have you marked as complete.  You do not need page 2. That is advice for K1 fiance's. The advice below does not apply to you because you are getting a spouse visa and will not file for AOS and you will not have to prove your shots ever again for immigration purposes.

Do you have to bring a list of medicines you take or have taken?  I managed to get a very brief medical report of any illness I have (asthma and eczema) and a few hospital admissions which were just twice for broken bones and a nasal surgery, but I don't have a list of medicines.

I'm getting my MMR shot and tetanus (or possibly td/IPV booster) on the morning of the examination at my regular doctor's surgery.  Do you think this will be ok?  It won't affect my blood test or anything will it?

No.  Funnily enough I asked about this at my medical!  I asked if I could have my bllod test before my jab in case it affected the results - the nurse looked a bit confused - 'I don't think it will matter' she said - and asked where I had heard that it might...

 

Anyway - I had my MMR and flu jab in the week bfore my medical and had another tetanus at my medical before the blood test - and all was fine.
