As long as you have all the vaccinations, and can bring your records/DS-3025 with you (I kept a copy) then getting them transcribed to I-693 is not too painful. It took me an afternoon, a little tenacity and $50. Though, maybe I was lucky? Or, not lucky enough to just let the little errors be ignored.

I think you were very lucky, just spoke to civil surgeons in my state... and they all saying I need a new medical examination done.

I'd read the USCIS policy manual, the CDC guidance and had a letter from USCIS stating exactly what to do. The junior/reception staff did initially try to get me to pay for an entire new medical but I was insistent. The civil surgeon took over and just did what I asked/had been asked to do by USCIS.

Maybe they just wanted to screw some money out of me? Or, perhaps, they are more used to work AOS applicants? I don't know - I just hope that USCIS accepts it as it's been submitted. >.>

I spoke to two day and they were all talking about doing everything over again.

I think I will rest my case till the RFE comes... I pray it doesn't. Lol!

I will still hope to hear from Knightsbridge.

Civil Surgeon don't read their memos or know the exceptions for a K1. Many refuse to listen to anything you have to say. I know the policies, memos, and versions of the I-693 from 2008 to present and made 10 calls one day just to advocate for a poor woman with limited English. It was so frustrating. Only one office had a woman who was interested and asked me to fax relevant copies of policies to her so she could learn that information. The rest were like "computer say no" and insisted their policy was full medical for any immigrant.

If the need arises check out the Civil Surgeon thread on this forum which can help you find a CS that is willing to just transcribe the vaccination record.

Fortunate indeed that you actually got to speak to the doctor. I was calling Los Angeles area from Texas and got a lot of stubborn people on the phone....receptionists, office managers, and nurses. And of course no way to speak to the doctor. Our own case was 2008 and we did not see a civil surgeon for anything when everyone else was afraid to try it, and were approved. The DS-3025 was changed fairly recently and the Results section is different. The instructions to panel physicians say now, do not mark anything in the Results for non-immigrant visa applicants. Leave it blank. So Knightsbridge followed instructions. Very recently (last month) they have added the note on the second page comments section stating the shots were complete, probably because so many in this UK forum have begged, pleaded, argued, etc. None of those people have advanced to AOS yet, so it is uncertain if that will help with hard-nosed adjudicators or not. The policy manual does state that if they can see the shots were given, they should accept them even if the doctor made mistakes on the results section. It's a toss-up whether you get a reasonable adjudicator or not.

I have applied for AOS already, should I send the new page to USCIS as an added-on?

No. It would never get placed in your folder? If you get an RFE send it or take to an interview. It may or may not satisfy them. It's not likely part of your original record which remained in a chain of custody that you never had an opportunity to alter so is it good enough? Edited June 9, 2016 by Nich-Nick

I've had my vaccinations for tetanus and diphtheria, but not for pertussis. Will I need to get the Tdap shot (tetanus, diphtheria and pertussis) when I have my examination?

Here's the clinic website with lots of information. http://www.visamedicals.info/us.asp

They answer their phone and can help, unlike USCIS or the embassy. Ring them up and and ask your questions about pertussis. Theoretically, yes you need pertussis if you have no record of a dose anytime in your life. Will they sign you off with just Td...maybe. I think they have for others. When my husband went under previous staff (2008), they were hard line, specifically on pertussis. They told him he needed the full tetanus/diptheria/pertussis because he hadn't had it as a child.
