Be sure and tell your GP that it is Tetanus/diptheria and pertussis. P is not for polio.

The vaccine for chickenpox isn't a regular vaccine in the UK. Because you have to be totally complete on your shots for an immigrant visa, you will get a waiver for the varicella shot marked "not routinely available" as the reason you are excused. In other words, they will still pass you for immigration without the varicella shot because most doctor offices and even the visa medical clinic in the UK do not have the shot to give you.

(Note to K1 visa people: this isn't true for you because you could still get your varicella shot in the US before your AOS since your visa does not require your to be complete.)

Thanks again Nick!

Hard to comment not seeing the actual documentation you have. On the American list, the p in Tdap means pertussis (whooping cough), so no polio vaccine is not a substitute. Polio is not even required for adults. Maybe you had the 4 in 1 shot with polio added to the DTP. I know they give pertussis vaccines in the UK to babies. If you had pertussis sometime as a baby, then just Diptheria and tetanus shot recently gets you through. That was in the one that also had polio. Maybe Knightsbridge will let you pass anyway without seeing proof of pertussis.

You can always pay at Knightsbridge for anything they say you lack. I know paying for anything medical is really weird to anybody UK , but it's a way of life to me so I would not hesitate to pay on the day if it got me through easy peasy. Remember Knightsbridfe always tell K1s they don't need to be complete. You may have to stamp your feet and insist you want all the shots for AOS done NOW! And you want your sheet to say they are all done.

Many thanks for the detailed response . Will see if my surgery will give me the missing shot( if applicable) the travel vaccination book I hold list , Tatanus /Diphtheria / polio . Will verify with the nurse if pertussis was included on my next appointment . Much appreciated

Do I need to make any preparations before the blood test?

I know some tests require a period of fasting beforehand and since it's my first test ever (never needed one) I'm a little in the dark.

No fasting. That is for glucose testing to diagnose and treat diabetes. That's not what they are looking for. The blood tests are to test for STDs mainly.

On the far side of this journey and have just had my AOS interview. Was approved on the spot but then got a surprise in the mail today asking for an I-693 with the DS-3025 form (you'll get a copy after your medical). Tragically, the doctor forgot to tick a box on my form (the important one saying "everything is cool, no I-693 medical report needed stateside) meaning I had to go find a civil surgeon, explain what I needed, convince him that, no, I did not need a full $400-600 medical examination and just to please to copy out and certify my vaccination report.

Pro-tip: get all your vaccinations BEFORE you attend the Knightsbridge appointment (GP does them for free) and get the travel vaccine document to prove your've had them. Then, after the medical, make sure the DS-3025 is filled out completely before you leave the Knightsbridge doctor's office. You'll save yourself time, money and stress!

Well that's the problem bariki. Like you, we received an RFE for the I-693, and like you Knightsbridge did not tick our DS-3025. The problem is that Knightsbridge literally REFUSED to tick any box at all on the form, and no amount of yelling could get them to budge. For a period of time Knightsbridge did every DS-3025 this way. Some got through their AOS fine without a box being ticked, and some did not.

Knightsbridge has now changed the form, and people are being given a 'page 2' with wording suggesting the vaccinations are complete (why I don't know.. they could save more trees by just TICKING THE DANG BOX). Recently people who were not given this page with their DS-3025s, have emailed Knightsbridge and they have responded by sending over a copy of page 2.

Whether applicants will or won't get an RFE for a I-693 is entirely up to the officer reviewing the AOS case.

Sigh, hopefully everything will be okay for the majority of those who had submitted without this additional page recently (including myself). I'm dreading RFEs for the AOS which I sent off last week.

I ended up writing a letter along with the DS3025 that I included in the I-485 pack. The letter was one signed page and basically directed the adjudicator to their manual and stating that it is the norm that the UK doctors haven't filled in the relevant box. I quoted the relevant parts of the adjudicator manual and am now just hoping that is okay (and that it doesn't irk them too much based on the suggestions I received in a thread elsewhere). I guess we shall see though. Anyway, here is a copy and I absolutely do not suggest others use this as a template at all because the new Knightsbridge letter might be much more effective. Let's hope that all will go okay for everybody who is going to Knightsbridge from now on!

I'm sure could have much tighter language and that there are errors in logic, grammar, spelling, errant words here or there. I also think I could have been a bit more pointed in my writing too. I wrote it a little too quickly and just hope I hadn't absolutely mangled nor misinterpreted the policy!

Subject: Explanation of Policy relevant to Vaccination Worksheet (DS-3025)

Dear Sir/Madam,

I believe that I am not required to have another medical examination at time of adjustment because:

  • I received a medical examination prior to admission as a K1 non-immigrant;

  • My Form I-485 was filed within 1 year of that overseas medical examination;

  • My medical examination did not reveal a Class A medical condition;

  • I surrendered the original Vaccination Documentation Worksheet (DS3025) as part of the

    sealed original, overseas medical examination report to the immigration office at my port of entry in [POE removed for anonymity]. I am under the impression that the original Vaccination record and the original, overseas medical examination report will be forwarded to you under separate cover. For completeness, however, I have attached a photocopy of my Vaccination Documentation Worksheet (DS-3025).

I am aware that the DS3025 has not been marked as complete in Section 2 of the form – this practice is the norm and standard practice for K1 visa medicals at the London embassy in the United Kingdom. Per the The Adjudicator’s Policy Manual, I am under the impression that failure to tick the complete box does not require corrective action. The section of note is in the Adjudicator’s Policy Manual (Volume 8, Part B, Chapter 9, Section F entitled “Completion of the Results Section by the Civil Surgeon”). The relevant extract is cited below:

“According to the Vaccination Component of the Technical Instructions, the civil surgeon should mark the appropriate results box at the bottom of the vaccination assessment chart. The Technical Instructions direct the civil surgeon to only check one appropriate box.

The officer should be aware that civil surgeons may improperly mark the boxes because they may misunderstand the meaning of these boxes. Therefore, the officer should determine, from the vaccination assessment completed by the civil surgeon, whether the applicant received all vaccines, which blanket waivers should be granted, and whether the applicant requires any other waivers. The officer should exercise discretion in reviewing the vaccination chart and when evaluating the results boxes at the bottom of the vaccination assessment chart.

If the civil surgeon did not check any result boxes, the officer should only return the form for corrective action if he or she is unable to ascertain whether the applicant is admissible. The officer should never alter or complete sections on the medical examination report that are the responsibility of the civil surgeon, such as the results boxes.”

Source: https://www.uscis.gov/policymanual/Print/PolicyManual-Volume8-PartB-Chapter9.html

I believe that the DS3025 that I have provided indicates that all the relevant waivers have been received, irrespective of there being no tick in either box in Section 2 of the DS3025 worksheet.

Yours faithfully, signed:

And whether or not said officer has read and understood USCIS' own guidelines on this matter! XD

Timeline

I have sent an email to Knightsbridge already, hoping to hear from them tomorrow. I am dreading RFE's for medical now.

Too bad they collected such amount of money and could not do a good job.
