packet3 checklist

Packet 3 contains the DS230 and a checklist both of which have to be returned to the Embassy. Can someone please tell me what is this checklist?

List of items needed for interview.

• police cert
  
• birth cert
  
• marriage cert (K-3)
  
• Passport
  
• I-134 (Affidavit of support)
  
• Military records, if available.
  
• Etc.
  

The list is called OF-169 or DS-2001 by the consulates, it is consulate specific, some consulates post it to their web site most do not.

ok I looked up the DS-2001. Now it says on Page 2 to put in Case Number-which Case Number the one from USCIS or the one from NVC? They are two entirely different numbers

Anyone know the answer to this?

NVC case number. This is what the consulates refer to, NVC and Consulate are both part of Department of State.
