For k1 I did medical in Canada but when doing AOS here now three months later I get conflicting information some people say i-693 is required and to see a civil surgeon others say that it is not. Could any one with in depth knowledge and experience weigh in on this.

I had the same confusion and followed the same advice given here. Along with a copy of my DS3025, I also included a cover letter which quoted the I-693 instructions specific to non-requirement to K1s filing within 1 month and specified the date and location of my medical.

What do you mean one month ? Can only do AOS within one month of medical done another country ? Please elaborate more as your answer is confusing to me

Ok so we don't have to see civil surgeon or do i-693? Do you know if it's same for people that are choosing not to get vaccinated  and will apply for a religious waiver ?

if that is the case you will more than likely be required to have a civil surgeon fill out the vaccine portion of the I-693 (not redo the whole medical). a lot of civil surgeons don't do that so you would have to look around and find one that would.

What happened in Canada at the medical? I have never seen a Canadian K1 not made to get all vaccines unless they already applied for the waiver. Did you get the waiver in Canada? They don't normally let the medical go to Montreal without you completing the vaccines. It would be good to know if just telling them you will be getting a waiver is enough. If you actually did do the waiver in Canada it may say on your DS3025. You need to look at the DS3025 and see what it says.

If your vaccination worksheet was ticked as complete, as @alliejourney advised, then you don't need either, but considering you have a separate thread asking for a vaccination waiver, I assume this isn't the case for you.

No you don't. If you are applying for a vaccine waiver based on religious convictions, all you send in is your AOS papers, your DS-3025 that is incomplete (mine was incomplete as I only received vaccines as a child) which you get a copy of from your CS in Canada you went to, and your waiver with your statement explaining why it is against your religious convictions, possibly statements from family members, friends and/or church members and you send all that together in one package with 2 checks (one for your AOS and one for your waiver) to USCIS. Unless you also apply for AP and EAD (I didn't because the waiver was costly enough!), that should be all. 

I was confused like you, it looked like I needed to see a CS here in the USA to just check the box that I would file for a waiver on form 693, but when I called they would only do that if I would redo the whole exam, which is ridiculous since I had it all done in The Netherlands. So I did not file 693 at all. USCIS already has my medical from the US, and the incomplete vaccine record is what I filed the waiver for.

I filed in July and still no green card so I don't have an update yet other than doing my biometrics, but this is how I did it and others before me who had shared it with me. 

I am assuming you are missing more than one vaccine?

Historically, one would submits their completed DS-3025 with their AOS packet without a I-693, but there has been some updated guidance regarding I-693 causing a lot of people adjusting status from a K visa to wonder if the I-693 is now required.

 

If all age-appropriate vaccines are completed, usually "K Visa applicant voluntarily completed vaccination requirements" is checked. One does not need to complete the full series, although panel physicians will usually encourage you to do so after your arrival in the US.

Are you choosing not to get vaccinated for COVID-19? If so, the newest USCIS Guidance is that AOS cannot be denied for lack of proof of COVID-19 vaccination. Similar guidance was issued by State for panel medical exams conducted for visas.

 

However, if you chose not to take other age-appropriate vaccines, then you may need a I-601 waiver to accompany the I-485.

 

The old and now invalid i-485 instructions dated 4/01/2024 stated that I-693 was not required:

You are NOT required to submit Form I-693 at the time you file your adjustment application, but may do so if you wish.  Because of the time-limited validity of Form I-693, you may choose to submit your Form I-693 after you file your Form I-485.  You may also submit Form I-693 in person at an interview in a USCIS field office, if an interview is required.  By waiting to submit Form I-693, you may avoid having to repeat the immigration medical examination.

 

The latest I-485 instructions on USCIS's website (dated 1/20/2025) does not include this paragraph and says that an I-693 is "generally required" for I-485/AOS, but the text is not definitive ("If you are required to submit a Form I-693").

A news release as of a 4 months ago (12/02/2024) is also not definitive. It says I-693 now must be submitted with a I-485 ("make filing the two forms together a requirement") due to the high volume of RFE requests. However, it also has the text "If you are required to submit Form I-693".

Applicants for adjustment of status generally must complete an immigration medical examination and all required vaccinations and submit a properly completed Form I-693 signed by a civil surgeon to show they are free from health conditions that would render them inadmissible under the health-related grounds.

If you are required to submit Form I-693, or a partial Form I-693 (such as the Vaccination Record), you must submit it with your Form I-485. Otherwise, we may reject your Form I-485. We have revised the Form I-485 instructions to make filing the two forms together a requirement.

We have made this change to reduce the number of Requests for Evidence we issue before adjudicating a Form I-485.

 

The I-693 Instructions includes a section for K visas that suggests that I-693 must be partially filled if the DS-3025 was not properly completed:

3. What if I am a K nonimmigrant visa holder and already completed an immigration medical examination abroad?

 

A. If you were admitted as a: A. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or

B. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

C. You received a medical examination prior to admission, then:

    (1) You are not required to complete another immigration medical examination as long as you file your Form I-485 within one year of an immigration medical examination completed outside the United States; and

       (a) The panel physician did not find a Class A medical condition during your imigration medical examination; or

       (b) The panel physician did find a Class A medical condition, you received a waiver of inadmissibility, and you have complied with the terms and conditions of the waiver.

    (2) Even if a new immigration medical examination is not required, you must still show proof that you complied with the vaccination requirements.  If the vaccination record (DS 3025) was not properly completed and included as part of the original medical examination report completed abroad, you will need to have the Part 10. Vaccination Record completed by a designated civil surgeon.  In this case, you must submit Parts 1. - 5., 7., and 10. of Form I-693.

 

I am guessing that an incomplete DS-3025 is the reason for the checkbox for Form I-693, Part I, Question 4.

If you chose not to get vaccinated for religious or moral convictions, does your DS-3025 have a box "Immigrant Visa applicant requests Individual Waiver based on religious or moral convictions" checked? Do you have an approved I-601 to accompany the I-485 assuming your refusal was for vaccines other than COVID-19? If the medical was conducted outside flu season, then the flu shot is not required (see here).
