Here, it is asking to provide the last address at which you physically lived together. My partner is coming and staying for couple of weeks, as they study at the university in different state. Also, we went back to our home country and stayed there for about 20 days, does that count?
 

 

"The beneficiary is in the United States and will apply for adjustment of status to that of a lawful permanent resident at the U.S. Citizenship and Immigration Services (USCIS) office in:". In this part, should I put my hometown, this is the place where we are planning to live after a few month, once he is done with classes? I live in Philadelphia and they live in Phoenix. However, my partner is planning to move to my address after a month of filing the I-130?

The form doesn’t ask about where you are living *together*, just about where the beneficiary lives  *right now*. 
 

Staying somewhere for a few weeks does not count as living somewhere. 
 

if you plan to move in together during the process, put the future address where both of you will be as the mailing address so that you know the mail will always reach you. Then, when the beneficiary moves, make sure you tell uscis their place of residence has changed.
