Vaccination record/form

Fiancee is planning to go to IOM clinic in Moscow for medical and will ask them to fill out the vaccination form DS2035 (which we will use it later for AOS).

She has all the vaccination records in various government medical record books that they maintain in Russia and she is in possession of these records.

On top of this, what she has done is that she has created a table of all vaccitionation that she has had with dates etc - basically copies all the information from those old records books on to this neat table and she has been able to get the

government medical clinic to verify it, sign it and put a stamp on it. Now this is not a form, but a tabular list of all vaccinations with records which been signed and stamped by the government clinic.

My question is - is the above enough to be taken to IOM clinic in Moscow and have them fill up DS2035? or is there any other format (other form?) that this data needs to be taken to IOM?

It could be written with finger paints on a napkin and be fine. The issue is the form being signed by a doctor that the US recognizes. It is up to the doctor to determine if the shots meet the requirements. You should be fine.

As long as there is a stamp and sign of a doctor its ok,though I would still check with IOM,i did there my medical check and since i lost all my previous records I did vaccination again.They have their own table of vaccinations you need to have depending on your age.so I would call IOM-they respond very clear and are very nice on the phone.
