I-130A for a child ?

I just received my appointment to file I-130 for my spouse and child. However the consulate is asking for something very strange.

 

• I-130A, Supplemental Information for Spouse Beneficiary and 1 photo (taken less than 3 months) 
• I-130A, Supplemental Information forChild Beneficiary and 1 photo (taken less than 3 months) 

As far a I know there is no such form as "I-130A, Supplemental Information for Child Beneficiary". What do I do ? Just file I-130A with my child as my spouse ?

 

Montreal is one of the slowest in the world, they take weeks to reply to any email so I have no way to ask them before the appointment.

This link leads to "Supplemental Information for Spouse Beneficiary" with "Spouse" clearly written all over the questions. They are asking for "Supplemental Information for Child Beneficiary"
