You have started two threads recently.

One is about a problem with the doctor in your wife's home country and now you have a problem with another doctor. Sorry if I have got that wrong.

The first thing I would do is clarify your concerns with clinic that filled out the I-693. They probably have a better idea of what they need to do than you, I or anyone on here.

You could find another doctor to do the vaccination section.

You could send off the paperwork you have and possibly get a Request for Evidence. (Lots of applications for AOS get them.)

Making an official complaint won't help the AOS application. That is not to say you should not do so if you want to.

Both problems are with the same doctor. The same Civil Surgeon in the U.S. He refused to accept my DS-3025 and said the U.S. Government doesn't accept it, he filled out the I-693 incorrectly and now they sent out the sealed medical incorrectly.

They do not have a better idea of what they need to do, they don't have ANY idea of what they need to do. I don't know what people keep thinking that. I pointed them towards the I-693 Instructions and the Technical Instructions for Civil Surgeons, both documents that they are required to follow. They did not care. They had no idea what a K-1 visa is and what the procedure for that is and thought I was lying about the whole thing. They did not believe me when I said that according to immigration law, I had to complete a medical in my home country and get my vaccines documented. I've been trying to clarify these concerns with the clinic for 10 days, they are simply not listening and do not care what I have to say. I want to file a complaint because it is not normal to take people's money, do things incorrectly and then refuse to do anything to fix it. We paid for a service and we did not get what we paid for. It seems like an open and shut case to me. The Civil Surgeon has to follow the official guidelines and rules, and he did not. I tried to point out these mistakes in order to get them fixed and they flat out ignored me.

Just to clarify, we will most likely be going to a different Civil Surgeon so there is no problem with the AOS

I am assuming there is some procedure one goes through to become a civil surgeon registered with USCIS so they must have SOME idea of what they are doing.  None of us can really comment on the service you received. It may be that in your part of the country they do not see many people adjusting from K1.

When I went to see a civil surgeon to get my last few vaccines signed off I did not show the nurse my DS-3025 from my K1 visa medical. She looked me up on the state immunisation database. Some one here, in another discussion, suggested including it when you send off your AOS packet.

I hope things go better for you with the new doctor!

My DS 3025 was sent to me after my medical by the medical provider in the UK by a secure link from which I could download it. I printed it off as well just in case. So I could check as well if it was marked correctly before travelling to the USA.

Did you receive your DS 3025 by email or by post?

Have you contacted the medical provider you did your medical with and asked them about the issue of your DS 3025 not being marked as complete? Not sure if they could update their records or not?

I would expect a CS to know what is needed to properly complete a medical, but nothing about Immigration, the various options etc.

I got a physical copy when the results of my K-1 medical exam were ready and I picked them up. It's been a while since my Medical, I just didn't know the correct procedure back then so I didn't say anything when I got my DS-3025. Would it be an option to just send the DS-3025 and the last 2 Hep B doses in with the I-485? Technically my DS-3025 is COMPLETE, the panel physician just incorrectly marked it.

So would I but unfortunately from the state of the envelope I received in the mail, I don't think they know.

I looked at the Form I-693 instructions and this is what I found:

Quote

Prepare the original of the completed and signed Form I-693 for submission to USCIS.

Follow these steps:

Place the Form I-693 and any supporting documentation into an envelope.

Seal the envelope.

On the front, type or print in capital letters: “DO NOT OPEN. FOR USCIS USE ONLY.”

On the back, write your initials across the seal where the flap meets the envelope.

Seal the entire flap with clear tape. Make sure the tape covers your initials as well as the flap.

Give the sealed envelope to the applicant.
IMPORTANT: USCIS will not accept Form I-693 unless it is in a sealed envelope that is not opened or altered in any way.

Do not accept the form from the civil surgeon unless it is in a sealed envelope. USCIS will return your Form I-693 to you if it is not in a sealed envelope or if the envelope is opened or altered in any way.

The envelope I received does not say have the "Do not open" written on it. There is no clear tape covering the doctor's signature. The envelope is open in one of the ends so it has been altered. I tried to add a picture but it does not work.

Are you not able to contact the place where you got your medical done and ask them about them marking the DS-3025 incorrectly?

I can only e-mail since I am in the U.S. right now but I do not think they would fix it. It was long ago and I do not think they would react positively to me contacting them after all this time to say they made a mistake. Even if they fixed it, USCIS has the original DS-3025 so I'm not sure how that would work out.

I tried doing that after I got some post-medical vaccines. I was told the form could not be amended after the paperwork had been sent to the embassy.

I would not just send in proof of those last two hep-b if you want to avoid a request for evidence.

If I were in your position I would just find some one to do the vaccine only portion of the I-693. Put a covering letter on top explaining why you have not done the full medical.

Makes sense - I didn't know whether it was possible that was all. I was able to get my Covid vaccine after my medical but before my interview they offered to hold the file as I had 6 weeks between my medical and interview so I could get it done. Once I had provided sufficient proof of vaccine they updated my file and sent it to the embassy.
