Vaccination supplement for AOS

Timeline

My spouse and her son has a copy of the Form DS-3025(vaccination documentation worksheet), they received when they had vaccinations done in India.

Is that adequate for the AOS filing?

Is there a special form for 'Vaccination supplement'?

At what stage is I693 required?

Is Form I693A used anywhere?

Please advise.

Hi there,

the vaccination supplement is form I-693A. It is required for filing for AOS, according to the instructions on the I-485. Of course, some people here have gotten away with just sending the DS 3025, others have gotten RFE'd for it. So it's up to you, whether you want to get it or not. I personally would definitely do it (and have done it).

The I-693 is the complete medical exam. Have your spouse and her son had her medical in India already (within the last 12 months)? If yes, then they won't need to do it again. If no, then you will need to schedule an appointment for them, have their medical done (only at a USCIS approved doctor!) and send the results in with the AOS.

If you do not need the COMPLETE medical, but just the I-693, then look up the USCIS approved doctors in your area, make some phone calls and ask around which ones would agree to transfer the information from the DS 3025 onto the I-693A. Some won't do it, so you will probably have to make some calls to different offices. They have the I-693A form, so you don't need to bring it. Just bring the DS 3025 and ID, and have them fill out the new form.
