Hello,

How to respond to this question: Have you EVER previously filed a petition for this beneficiary or any other alien?

We used in the past a lawyer he filed an i130 petition and we don't know the outcome since I left the country while on AOS and lost contact with the lawyer in order to know the outcome. And we have no reference of the i130 receipt number.

If a I-130 has previously been filed for you, then the answer is yes.

I believe you can do an INFOPASS to go to your local office and ask for the case number if it exists

That could be a way out

You can simply enter "unknown outcome" in that space.  But, if I understand you correctly, you left the USA and abandoned an adjustment of status process.  If so, the outcome was, Adjustment of Status abandoned.

Yes I left while AOS pending. But I finally found the i130 receipt. I checked it online and it was denied because there were RFE that I didn't replied to. And I submit a FOIL to get the detail of the i130 and i485 denials, so I am waiting for it.

That's the only answer you need.  Denied because no reply to RFE.  They are not asking about anything but the I-130.  That is the "petition".  You can be certain the I-485 was denied too, as it cannot be approved without an approved petition.  But there are no questions about the I-485.
