I received an email stating that the documents were returned and that I should collect the passport from theircollectioncenter. However, my case status has not changed to "issued" or "approved" but is marked as "ready," while my husband's status shows as "refused." This could indicate that his visa application is refused?

Always mention the country/embassy

this is Islambad so no one can answer

Did u have to do the 5535 u were worried about as that takes time

Refused is what all petitions say until u see denied or issued on CEAC

 

And at this time with US embassys closed for all holidays and then Januaary 9th in honor of former President Carter don't expect CEAC to really be up to date 

USCIS and CEAC updates are slow (if they slow, slow, slow)

the  system lacks good communication with petitioners

Yes, we have already submitted 5535.

Refused doesn’t mean denied - it means he’s in AP. So just a case of waiting that out.
