Hello!

 

My wife's interview on 12/23 went well, however, we (stupidly) placed my original divorve decree in another folder that stayed home and she was given a 221g letter and she was told her visa would be approved once they receive the original missing document. The instructions are very straight forward and we're not really worried about anything, however we are having trouble finding this "Document Submission Form" they request. The link in the letter doesn't go anywhere that has that option. We did several searches and find other versions if this document from international US embassies, just not the one for Manila. Would someone here have a copy of it or a working link to it?

 

Also, we read on the Embassy website that instead of sending it via courier, we could submit it in person a the VAC at Parqal. We will be staying in the area next week and plan on stopping by but don't know if we need to make an appointment for this. Has anyone submitted docs directly this way?

Your visa application is refused under Section 221(g) of the Immigration and Nationality Act (INA). It is possible that this denial may be overcome,if you provide everything requested in this letter.
