It is so sad that we have reached part 4 of this thread. I'm wishing this madness comes to end soon.

Question:

I am sending my passport to montreal consulate for visa stamp via purolator .

My question is should i pick delivery with  signature required service or without signature please guide me if anyone had used purolator in the past for sending passport for visa.

We contacted senator office, and they sent the email to consulate and today consulate replied that they have not received my passport and photos yet because all mail goes to offsite facility which takes additional time to reach visa unit. It is so surprising that their process of getting passport from offsite facility to visa unit is so slow. As UPS notified that Steve received my passport at off sight facility of consulate on Nov 27. It is almost 3 weeks after that, I am still waiting for my visa to be issued.

It was consistently reported on this thread that visa issuance after the postage of passport, pictures and electronic transmission of the medical takes 2 months to be processed by the consulate in Montreal. Nothing you can do but wait.

I have a visa appointment at the end of January but since I am from Iran, the consulate already emailed me and asked to submit DS-5535. Since this is going to take at least a few months, to avoid paying twice, I am wondering if I can do the medical test after my visa appointment when the AP has been completed?

Is your primary nationality and passport from Iran? Are you also a Canadian citizen? Or is Iran the only nationality?

 We know that medical has validity of 6 months and police clearance of 1 year. 
And visa has to be issued before medial expires, else medical needs to be redone. Is it same for police clearance as well? Are there any cases that after AP has been cleared and passports have been submitted, and MTL asks to redo police clearance if 12 months have passed since previous police clearance?

@KDEE yes I just received the instructions that I have to send a new police certificate with the new medical. I think the police clearance is valid for 2 years but not 100% sure. Mine was from 2.5 years ago, my case is not DS5535 but I-601 waiver which took 20 months to be approved, then 4 months to receive the follow up instructions from Montreal.
