Hello all I was just posting because I had a question. This month my fiance and I will be getting married and I will be submitting the adjustment of status paperwork immediately afterwards. Looking through her records I only have the vaccination record from St Luke's. The missives state that another form is required verified by a civil surgeon. Form I-693. So I'm a bit confused as to whether this was already completed in the Philippines and I lack the paperwork for some mysterious reason or if this requires another medical examination here in America. If anyone can provide clarity I'd appreciate it

To add more fuel to the fire ice read the instructions and they are saying if foreign medical was completed within one year we should be good to go. I am thoroughly confused at this point

your fiance should have received a form DS-3025, Vaccine Documentation Worksheet, showing that she received all required vaccines from the doctor who did her medical. she would have gotten this at the end of her medical

it is best practice to send a COPY of this form along with the adjustment of status packet.

 

but yes, the I-693 is not required for K-1 visa holders if filing the AOS packet within 1 year of the medical exam date.

 

The envelope that she gave to border patrol at her POE had her medical in it, that information gets sent to USCIS, so they should already have it.

 

sending a copy of the DS-3025 is just to show that she did complete the medical. cause sometimes it can get lost between border patrol and USCIS

 

if she didn't get this form, she can try contacting where she had her medical to see if they can email her a copy of it
