One dose is required for AGE-APPROPRIATE vaccines. You'd have received a blanket waiver for it for B. Insufficient time interval to complete series.

You'd also have received a blanket waiver for this as D. Vaccine routinely unavailable if truly unavailable.

for me hepatitis is required until 59. I will be 60 in January. So there was no point in having two doses in a rush.

That's what blanket waivers are for.

Are you SURE your DS3025 is marked  INcomplete on the left bottom of the FIRST PAGE?

It would only be incomplete by your doing and CHOOSING not to get vaccines. For ones not available, not age appropriate or out of season etc, you would have received a waiver and it would be marked complete AND would be complete. 

 

What is your's marked ?

And why would it be marked that?
