Medical exam of applicant for family based petition

Hello, Thank you in advance for answering. The question I have is that my niece is here in USA under student visa and currently enrolled at a university. However, my mom petitioned for her dad and she is also an applicant/dependent. NVC has reviewed and accepted all the documentations and now they are at Step 10 and waiting for interview appointment (Step 10 details are here: Interview Preparation. ) My question is regarding the medical exam, since she is in the USA legally at this moment, can she schedule the medical exam here or does she have to fly back to her home country for the medical exam part? They have yet to receive the interview appointment letter but we are being proactive on this. Since it takes time to schedule medical exam (I personally have done it as part of my green card years ago so I know that it could take days to get an appointment) so we want to be prepared.

According to my knowledge, ur niece can apply for adjustment of status once her parents receive greencard or citizenship

Thank you. But the biggest concern now is that she will be 21 in April and I have had similar experience where when I applied for AOJ, it was denied. They had told me that once I turned 21, I could no longer be in the same visa category. As of now, their interview window is around 7-9 months.
