CEAC Checker IV Issued

I have a silly question lol, I was just wondering if anyones CEAC checker indicated "issued" but then didn't receive the visa in their passport. 

This hasn't happened to me but I'm curious as to if it can happen. 

If you're refused does it just stay as refused, and they mail you your passport back? Does it changed to denied?

If it's issued then it means the visa has been printed and is in your passport. 

 

And yes, it can stay refused whilst in Admin Processing, and that can then go to denied. You can ask for your passport back at any point if you need it.

so it would never say issued if it in fact, was not issued?
