That statement was for non-immigration-required vaccines, such as for school entry.  For immigration-required vaccines, they must be administered by a civil surgeon, and they will likely perform the full exam.

 

You said your DS-3025 is only one page...  does it say "Please Complete Page 2" and "Page 1 of 2" on the bottom?  It's been 2 pages for several years, so I would be surprised if you got an older, single page one.
