K1 I-129F Question for Province on Form

Hi, I cannot figure out how to fill out the province and the city for my fiance for the I-129F.

 

The city is Varzea Amarante, I do not know how the formal way to fill it out on the 129F form would be be can someone please help me

 

Would it be like this?

City or Town:

1) Varzea, Amarante

2) Varzea AMT

3) Varzea Amamante (without the comma)

 

Province:

Does anyone know what to put here is this Porto?
