The interviewing CO "approved" both of our IR5 applications, kept our passports and told us that we will get our passports back with visa in 6 days. Wife did get her visa issued on 1/16 and received her passport back a few days later but my status changed to "refused" when I first check CEAC on 1/17. Since then, another 4 date updates on 2/1, 2/2, 2/7 and 2/13 (last update) but status remain as refused.

 

My bigest concern is that I never received any instructions from the CO during and after the interview. No 221g, no DS5535, not email or regular mail, nothing at all from the MTL office. But the refuse note on the CEAC status page states "A U.S. consular officer has adjudicated and refused your visa application. Please follow any instructions provided by the consular officer". I am concerned that the CO instructions for me got lost somehow on the way.

 

Is there anyone here having the similar experience? Any suggestions on what I should do now will be greatly appreciated.

If you have not received any email from consulate after interview. It means consulate is checking something on your application. Do you have any issue or violations when you used to live in USA. I know one case where mother of US citizen entered US illegally 20 years back and CO refused her application(221g) and sent her email to explain that (whether she committed fraud to enter US or just crossed border as illegal immigrant) but she was issued visa after 2 months.

Anyway it is good sign and just wait and keep sending them email every 2 weeks as you do not know the reason.

No, I have not had any issues or violations when work in USA, not even a traffic ticket. I am working in USA with a TN visa and recently have my TN renewed. I used to commute to work from Canada for the last 20+ years but stopped crossing when my company filed I-129 to renew TN for me in early Feb. this year and stayed in USA ever since then. I am wondering if TN (non-immigrantion intended visa) is the source of the problem.

It’s fairly straightforward.  Your Personal Information, Visa Type/Classification, Current Passport #, email address, your MTL Barcode#, Passport and Travel (they’re asking for information to any country you travelled to … other than your country of residence …  in the last 15 years) history. They’re looking for locations visited, date visited, source of funds for the trip, and length of trip. For Passports other than the one listed in your visa application that you’ve previously held, they’re asking for country of issuance and passport numbers. 

 

Relatives, siblings, children and spouses - names, DOB, living or deceased.

 

Addresses lived, and employment history over the last 15 years, if not already provided in your application.  Type your name, and date.  Lots of fun!

In regards to travel history, what format do you recommend? tabular or list? should we include all the 2-3 days trip to US too?

You need to include ANY and ALL travel/trips outside your country of residence going back 15 years, regardless of length and yes, that includes the US.  As to tabular or list, I would say any way you can make it fit in the allotted space.  My husband traveled extensively and had many, many travel entries.  He saved the information in a separate document then did a cut and pasted job.  Good luck
