Request for Initial Evidence on I-485 based on Approved EB2-NIW

Title. I'm a Turkish PhD student applying on I-485 based on approved EB2-NIW.

 

Even though I didn't get the RFIE in mail so I don't know if they sent the RFIE for my birth certificate, I'm fairly certain it's the reason because I submitted my sealed envelope medical exam and everything else. 

I sent Formula A for my I-485 (EB2-NIW based). Can it be the reason? I went to the Consulate and they gave me a document with blue seal, with English translations, and said they give this to everyone applying for greencard.

 

They also attached Vukuatli Nufus Kaydi, the same on e-devlet but in Turkish and told me to send it anyways.

 

Other possibilities are, my medical exam had Insufficient Time Interval for Polio and TDAP vaccines as I got one does and the second and third one were separated by some time apart. My civil surgeon said 1 dose is sufficient for USCIS purposes.

 

I'm doing a summer internship so I got an employment document from company and not my school, so do they need a document confirming my continued employment for PhD?
