Help my weary mind..What all does Ann need for medical and interview?

Yikes, amazing how the mind starts going....

She will have the following:

For the Medical:

Passport

Appointment Letter

$95 Peso Equivalent

Passport Photo

Ink Pen

Don't have to worry about immunization records.. cause there ain't any...And she hates needles, could be a fun day.

(Can she take her cell phone into St. Lukes?)

For the Interview:

I-134 (2 Copies), with 2006 Tax Return, 2006 W-2, Statment of income and employment from my company, Letter from my bank, and a couple check stubs from work.

DS-156 (2 Copies) Filled out online, printed and signed by Annie.

DS-157 (2 Copies) Along with payment validation stamp from the bank (BPI Angeles City, Main Bank)

DS-156K (1 Copy) Filled out online, printed, and signed by Maganda Girl.

Passport?? - I thought this was taken at St. Lukes and forwarded to the US Embassy..

Medical Results??? - This is forwarded by St. Lukes, also, right??

Birth Certificate - NSO

CENOMAR - Certificate of No Marriage for Annie.

NBI Clearance - (She is a cute little criminal..)

Evidence of Meeting - Hotel Receipt, Cebu Pacific Boarding passes (for both of us, side by side), Northwest Boarding passes, pictures from both trips. (Evidence needs to be the originals, correct?)

Evidence of Engagement - We've chatted forever on Yahoo, and talked on the phone for 1,000's of hours, but not much snail mail or email. I'll select a few good emails, a few good archived chats, engagement ring receipt, phone records showing 1000's of calls. (Anything else?)

Proof of Termination of Marriage (From me) - Certified copies of Divorce Decree from court house.

2 Passport Photos.

(Leave cell phone with cousin, before entering Embassy.)

February 13th 2006 - Met in Real - WOW!! Ikaw maganda!

February 14th 2006 - Proved I was really serious, proposed to Annie - She said "Yes"

February 2006 - January 2007 - Both working, saving, planning. Continuously chasing Annie online, on phone...

January 22, 2007 - February 4th, 2007 - Together again in Philippines, working on paperwork, visiting family.

February 27th 2007 - I-129F Mailed to Nebraska

March 6th - NOA1 Received.

May 23rd - NOA2, Received Approval Email

June 14th - Receive letter from NVC, packet on the way to Manila Embassy

July 30th - Annie receives packet from Embassy, appointment scheduled for September.

August 29th - Annie survives Blood Draw, Injections, and successfully completes Medical at St. Lukes.

September 13th - Annie is a huge success at the Embassy, SHE IS APPROVED!

September 20th - Visa in Hand, tickets purchased..

December 18th - Jumped on Airplane, Philippines for Christmas and New Year's.

December 27th - CFO Seminar Succesfully Completed

January 8th - Jumped on Airplane to USA together, Seattle POE, arrived home to Kansas.

March 14, 2008 - Married

May 19, 2008 - AOS NOA1 Recieved, Biometrics scheduled for June 18.

You have covered everything.

For the Medical - Yes, she can bring her cellphone there. Three 2x2 Photo (write name and MNL number at the back). No need to bring immunization records if there is none. If she have had chickenpox, she only need to show the pock marks (scar) so Varicella vaccine will not be given to her anymore.

For the Interview - you are all set and ready for the it. For the documents of proof of relationships - she can bring the originals, it will be returned to her after the interview. All her documents (NBI, Police Certificates, CENOMAR, BC, others) should be original copies. The passport and medical results will be forwarded by SLEC to USEM for her interview.

03 March 2010 - USCIS Acceptance Confirmation via e-mail and text message

06 March 2010 - received I-797C, Notice of Action/Receipt

01 April 2010 - Biometrics Appointment (Biometrics done 4/7/10)

27 April 2010 - received I-797C/ Request to Appear for Naturalization Interview

02 June 2010 - Interview schedule

17 June 2010 - Oath Taking (Memorial Auditorium, Sacramento, CA)

Help me out here...

How many total passport photo's does she need?

Are the 3 at the medical sent on to the Embassy, or is it 3 at the medical and a couple for the Embassy?

If it's going to be, it's up to me!
