How to understand exactly why document was rejected?

Hello,

I had my birth certificate and India police certificate rejected by the NVC. The notes are a standard response saying please upload an acceptable doc from correct authority.

I've checked the reciprocity page before submitting and did it again after this was received. My original birth certificate has the name, date, signature, seal and was dated a few days after my birth. I really can't figure out why it was rejected. 

My police certificate has all the details and the police commissioner's signature and seal as well.

Has anyone faced these? What did you resubmit and how did you get DQ after that? It seems even on this forum people ask the questions and then once they get DQ, they disappear without sharing what they did differently to succeed.  Please help if you have any tips.. thanks!

Also, if I resubmit the same documents (these are originals and the only ones I have), with an explanation in the comments,  Would it be sufficient? Or do I need to write a letter explaining and upload it as an additional document? What would you advice?

Hi! I’m not sure if I can actually help you but are your parents names etc on your birth certificate as well? And I can assume it’s in English next to being in Japanese?
sorry if that’s not helpful at all but these were my thoughts why it could be unaccepted

Timeline

Resubmitted the following and got accepted and documentarily qualified.

Birth certificate: got the original notarized and apostille

Police certificate: got it issued through the embassy with their seal (probably certificate direct from the local police commissioner's office didn't match the 'issuing authority' mentioned on the reciprocity page)
