Vaccination/Immunization History from GP

My GP printed mine off there for me, just because I asked as I don't have a printer at home. It's not signed in any way but lists the following:

 

  • Who printed it (surgery receptionist's name)
  • The name of the surgery
  • My full name
  • "Patient Summary: LD - Vaccinations"
  • My DOB, gender, language details
  • My contact details
  • My surgery registration details (my usual GP, surgery address, etc)
  • All my vaccinations to date of printing, starting from earliest to most recent

 

I think if what you received via email has this info, you should be fine to just print it yourself.

For the medical you need your 'Summary Care Record', is that what you've got? Should be fine if so, mine did have my surgery name on the top right hand corner but I can't imagine it would matter if yours didn't have that on.

Yeah, just vaccination history won't be enough. You also need medical history, any medications etc - all of that (along with the vaccination records) will be on your SCR. Have you booked your medical yet? As the confirmation should tell you what you need to bring.

No, I'm far from the stage of booking any medical. I reached out to obtain my vaccination records to see if I could get missing ones done privately elsewhere for cheaper rather than at the medical. We are one adult and 2 children, so it's going to be a significant cost.

Ah, I see. You'll be sent a full list of everything that you need to get before the appointment then, I wouldn't get your SCR yet as they may say it needs to be more recent when you do have the medical. 

 

Your GP surgery should be able to do most missing vaccinations.

The medical secretary at my practice couldn't find my full records as I've moved around in the past, including overseas, (although that might not be why they are missing) but anyway she came to a standstill on it so I made an appointment with a nurse at the practice and asked her instead. She printed out the patient care summary immediately, for free, which only covered 2012 onwards. I emailed Visa Medicals to double check and they said it was okay that i didn't have full records, to just bring what i had and they could take it from there. This could be because i have no ongoing medications or conditions, but i would advise a quick email or call to Visa Medicals if you are unsure. They can even do a blood test on the day you visit to determine which vaccines to give you (i didn't have that though). What i did have however was old cards from when i was a child that showed my vaccinations, which they made copies of. At no point did i need any kind of signed document or doctors signature. I've read from others on this forum that if you have an ongoing condition its best to get a doctors cover note/letter to give more details, outline treatment, and support you are not a danger to yourself or others etc.
