On the second page, does the blacked-out name and date match that of your 129F petition and petitioner? The first page is definitely asking for your valid marriage certificate. If you sent a photocopy the last time it might be worth getting an extra certified copy to send this time (in my county the copies are cheap enough to warrant this).

 

Honestly, might be time to get in contact with an attorney. They might be able to help you navigate this in the best way to save your case.

Yup did that and just got approved yaay!!  But thank you for your advise
