DS-160 question for Filipino with 2 first name and a different Middle Name (Mother's Last Name), should I put it all on Given name?

In the Philippines, our naming convention differs from that of the United States. Here, our middle name typically comprises our mother's maiden surname, as opposed to the American practice of utilizing a second first name or a middle name.

In my particular case, my name structure consists of two first names, followed by a middle name (which is my mother's maiden surname), and finally, my last name.

When completing the DS-160 form, should I consolidate all components of my name under "Given Name"?

For instance:

Surname: Lopez
Given Name: Daisy Jane Cruz

Yea, the US doesn't really have an equivalent to Filipino middle names, so it all goes as given names.  Your example is correct.

Just follow what you are accustomed.

GIVEN NAME: DONATO RAY

MIDDLE NAME: NAVALES

SURNAME: BONITO

FULL NAME: DONATO RAY NAVALES BONITO
