hello all,

me and my wife are filling out adjustment of status forms, I-485.

unfortunately, she never received a vaccination supplementation form(I-693A) from her physician abroad during her k-1 visa medical examination. however, the brown envelope given to the immigration officer at the port of entry contained her medical examination and this vaccination supplementation form, as stated by the officer.

so do we really need to turn in the vaccination supplementation form, I-693A, along with the adjustment of status papers, I-485. if so, where do i get this paper from since my fiance never received it?

recommendations/suggestions? thanks.

You dont get a 693A from doctor abroad, you get that from a US Civil Surgeon here in the USA.

My Wife was vaccinated in China before coming to the USA, and they gave her a book with the vaccination record, we then took this book to the Civil Surgeon here, and he transfered it to a the supplement (I-693A).

Search this site for "693A" and you will get all the answers that you need.

She wouldn't have received an I-693A from her physician abroad. Aside from the fact that there is no form designated I-693A, the panel physicians overseas do not use the USCIS I-693 medical report forms. The vaccination form used by panel physicians overseas is DS-3025 Vaccination Documentation Worksheet.

You could try including a sheet in the I-485 submission package stating that the K3 medical examination forms, including the vaccination supplement, were turned over to CBP on entry to the USA and have been forwarded by CBP to the USCIS for inclusion in the case file. (I'm intentionally using the USCIS terminology here, although I corrected you about it in the 1st paragraph above, because it's a USCIS clerk or case officer who will be reading this statement). If you are lucky this will get you past the clerk who opens the submission and is comparing the contents against a checklist of things to look for in the submission.

If you're not lucky you will get an RFE, which you may get later on anyway, and have to visit a Civil Surgeon to get an I-693 Vaccination Supplement.

Good luck on the AOS. :thumbs: When you start looking for a civil surgeon make sure you call around for pricing, it can vary by a wide margin.

Hello, I lived in Montgomery County MD and I just had my I 693 done last month.

Just look online for a civil surgeon in your area. Make sure u called a few and compare prices. I pay $ 220 and I had every thing done.

The Dr. has the forms and the medical exam is a breathe he was with me less than 10' . I had my nice seal envellope within 2 days.

Hope your wife enjoys DC regardless of the heat wave.... such a beautilul town.

Correct, you need to be very firm and explain that all you need is the vaccination supplement, most of them like to quote a full medical. Ours was $35
