Form 221(g) do i need to send passport?

On the interview for my case IR5 the counsellor took my passport and the interview seemed like it went well, on the same day after the interview I received an email from the US embassy to fill form DS 5535 and email it back, so i did that.
 

A day after.I received my passport back by mail without the visa printed and B1/B2 visa cancelled. Included in the mail was form 221(g) requesting to provide documents “passport” checked

 

Does the embassy need me to send my passport back? 
I am confused as to why did they send back my passport at the same time asking me to send it back to them? 

did this happen to anyone before or any opinions ? 
I have attached the form for reference.

My speculation is that they wanted you to see that your B2 was canceled (without prejudice).  "Okay to Issue" looks like they just need your passport to issue the immigrant visa.  Congratulations.

In summary, it took 13 months for approval of the CR-1.  It took 44 months for approval of the I-751.  It took 4 months for approval of the N-400.   It took 172 days from N-400 application to Oath Ceremony.   It took 6 weeks for Passport, then 7 additional weeks for return of wife's Naturalization Certificate..
