I am a US Citizen my wife I130 was approved and my wife gave birth to my son need help. My wife is in Dominican Republic my son is about 3 month old what proceeds to now that we started the nvc process I just payed the fees I am questioning about the process do we need to add him to the nvc application? Does she need to take a dominican passport for him over there or what proceeds? Since I am a US citizen I know he will eventually be a us citizen just dont know what to do?

He might already be a US citizen. 

1.) Where do you currently live?

2.) Have you lived in the US before? For how long?

I am in new york my wife in Dominican republic I am petitioning her I-130 so she was pregnant while the I-130 was in proccess so it was approved about 4 month ago so she gave birth to my son around that same time the case is now in NVC my question is what we do with adding my new child when he wasnt in the I130 application how does it proceeds now?

If you're a US citizen and meet the requirements, you'll do CRBA for your son https://do.usembassy.gov/ecrba/

 US citizens are not eligible for immigrant visas.   Have you looked at the link provided, as suggested?  You need to start there to begin to understand this process.

If your son is a US citizen you can include him in the DS260 to show your wife (and you) have a child.  

As you are a US citizen petitioning your wife (an immediate relative), your son cannot be added to the case so he receives a visa.

If your son is not a US citizen. You will have to file an I-130 so you can move to the US.

Timeline

First, determine if your son is entitled to US Citizenship at birth.  If you are a lifelong US Citizen, just assume he is.  What you do is apply for his Consular Report of Birth Abroad (CRBA) and his US passport.
