*** Moved from Bringing Family of USC forum to AOS from Work/Student/Tourist Visas forum -- topic is about AOS of parent ***

You're supposed to list all the legal names that the beneficiary ever used in their life.  Under "Beneficiary's Full Name", list your mother's current legal name.  This could be the name listed on her current passport.  Then your mother's maiden name (and all other names she used in her civil documents) should be listed under the section for "Other Names Used".

Because she changed her name at least once in her life, you must also submit a copy of her legal name change document, along with the I-130.  Examples: marriage certificate, divorce decree, adoption decree, name change court order.

Thank you, do you know, if I need a proof for legal name change for each form, i-130 and i-485, so I have to iclude two copies of marriage certificcates for each form, or 1 for i-130 is enough, as I am submitting it all together?
Thank you!

Treat each form as “stand alone”   Attach all required documentation to every form. Different forms are processed at different centres. Don’t forget to also apply for the travel permit and work permit at the same time as the AOS. There is no fee for b these when filed with an active I 130

A little confused.  The OP seems to indicate the first petition was filed for the son, and now the new I130 is for the mother (I assume there was no previous petition for the mother).  If that is the case, you would indicate the son’s information in Part 5, and the city where you live(d) when you filed the I130 for your son in your mother’s I130.  I don’t think the fact that the mother is in the U.S. and planning to AOS is relevant here.

Good Luck!

Okay. For question 5. Put actively being reviewed if USCIS didn't make a decision on the 1st petition you filed.

Look up the instructions for the I 130 to understand if the place you should put is your state or the location of the beneficiary you filed the 1st petition for.
