Civil surgeon and sign off

So I have my ds 3025 with my vaccines and only have a couple to tick off before AOS. I’m getting them done next week and when I go to the civil surgeon in the US should I bring a copy of my GP updated vaccines and my ds 3025, will that be good enough for the surgeon to sign or do I need something specific showing I got my updated vaccines after the medical?

You will bring the DS3025 and the record of any other vaccinations to the civil surgeon and they will transcibe onto the I693a for you.

 

Out of curiosity, what is marked on the bottom left of the DS3025?  Is it marked complete or something else?

k1 visa applicant electing to not be vaccinated at the time of examination

So when i do get the required vaccines, i just need my updated vaccination record and being my DS3025 to the civil surgeon then he will transcribe it for me for AOS?

You have to make sure that the civil surgeon you visit is willing to recertify the vaccine portion only instead of giving you an entirely new medical. You would need to bring ID, all proof of vaccines you've received, the DS3025, and the filled out I693 (required portions only). perfect thank you, luckily my fiance lives in a city with a lot of civil surgeons so im sure we'll find somebody
