Documents for Interview, Dual citizenship with different names.

My wife's interview is around the corner and I have a few questions.

 

CR-1

Tokyo Embassy

 

1. Do I need a physical letter from the Embassy about the interview appointment or do I print the email? 

I have 1 email from NVC for Interview appointment but haven't received any physical letter yet.

 

2. Do we need any documents for dual citizenship with different names?

My wife was born in Japan but had dual citizenship because her mother is from a different country. My wife has different names in each country.  Japan name is "Sakura, English" but used to be "Sakura, Toyota".  In mothers country, her name is "Saraku, Himari".

 

The embassy is asking for "Original Legal name change from Himari Saraku, to Toyota Sakura with English translation"

 

we have her original Japanese family registry that shows she was born in Japan, and her name change from "Sakura, Toyota" to "Sakura, English".

 

She never got a name change in her mothers country and never registered our marriage.
