More realistically this may well be a case where a Lawyer would be advisable.

In the first employment related field, enter Disabled Veteran and provide a start date.  If that start date was  more than five years ago, you're done with that section.

My mother-in-law had an original birth certificate in her Chinese-based name, and then a name change document when the country's government decided that Chinese-based names were bad and everyone had to have locally-based names.

 

I had to provide both documents.  I think I put her name change document under the Supplemental section.

 

Some people use records from school or their church to prove their date or place of birth.

 

It is not just for children.  If you have no supplemental records, you can ignore it.

Ok so when I went ahead and it said that if I don’t have any of these records then I have to submit a written document from 2 witnesses that knows about it. It also said that if I don’t submit it it will result in my application being delayed or not processed.

Don't continue until you figure out what's wrong.

I seeing the information and you explained it to me so what I’m going to do is submit her birth certificate that includes her baptism and religious records included like you said
