hi everybody,

My wife is here on K-3 visa.

anybody knows which/what form to use for the Tuberculosis (TB) test result?

I dont see any TB portion on the I-693A supplement form.

Will the civil surgeon use the I-693 form for the TB test?

Or will the civil surgeon issue a medical certificate regarding the TB test result?

When I went to get my TB test, the doctor gave me a bunch of forms to fill out and he had his own set of forms he filled out as well.

The part where the CS will fill out the info regarding the TB skin test is on the I-693 form. the supplemental form (I-693A) is for the vaccination thing only.

June 22 '07 received email saying CSC got our RFE and processing resumed on our case

July 7 '07another RFE for TB skin test

July 11 '07 sent RFE back

July 20 '07 case processing resumed per email from CRIS

July 24 '07 Email from CRIS...card production ordered!!

July 25 '07 received email saying letter welcoming new permanent resident was mailed

If you are required to submit a TB skin test the civil surgeon will write his own report, which will be given to you in a sealed envelope for submitting to the USCIS. On the Appendix A: Supplemental Form To I-693 (known as the I-693a here on VJ) there is no TB vaccine area to be filled in, hence that's why the civil surgeon will supply his/her own forms for the test results.

I've been looking but haven't seen anything either. There was one person but then her IO's supervisor said it wasn't necessary (I believe you saw that thread as well). That's all I've seen really for adjustment applicants who entered on a K1 or K3.

2007-04-16: I-485 and I-765 sent to Chicago (My AOS/EAD checklist)

2007-04-17: Received at Chicago

2007-04-23: NOA1 date (both)

2007-05-10: Biometrics appointment (both - Biometrics review)

2007-06-05: AOS interview letter date

2007-06-13: AOS interview letter received in mail

2007-07-03: EAD card production ordered

2007-07-07: EAD card received! (yay!)

2007-08-23: AOS interview (Documents / Interview review)

2007-08-23: Green card production ordered!!!

2007-08-24: Welcome notice mailed!

2007-08-27: Green card production ordered again... ?

2007-08-28: Welcome notice received!

2007-09-01: Green card received!

Done with USCIS until May 23, 2009!

Every doctor my wife called about getting a new medical said she'd need the TB test. Maybe it's a requirement after that scare from the guy who went all over Europe with it????

CIS Office : Philadelphia PA

08/31/09 Check is Cashed/Clears

08/27/09 NOA

09/24/09 Biometrics

11/24/09 Approval letter arrives!

Nothing USCIS does is consistent. The CS here locally required it.

I had my medical yesterday and the doc did a TB skin test. I'm not sure if it's a requirment for the interveiw, but the civil surgeons are certainly still doing the TB skin test.
