Quick questions about online submissions

I appreciate this forum so much and thank you all for helping people that you don't even know without expecting anything in return. Process is stressful and this support makes it easy.

 

My questions about my submission is:

1 - When online form asks about "beneficiaries family" it only has options to choose for "spouse" and "child". I put myself(spouse of beneficiary) as 1 item, however do I need to enter my wife's(beneficiary) siblings and parents? Or do we only enter "spouse" and "child" as online form is forcing us to choose? Has anyone entered other family members? (I attached screenshot for this section)

 

2- My wife changed her last name after marriage. Lets say her full name was "ashley brown", not her name is "susan white". All of the documents(Passports etc) has her new last name(White). Do we need to upload proof of name change as it asks in "proof of legal name change"? It says "if either you or the person you are filing for is using a name that is not the same name shown on documents...." Most important documents show her full new name. But we have documents like hotel reservations we had before marriage, so it has her old last name. Do I still need to upload proof of legal name change for this? I hope my question is clear. I also attached a screenshot for this.

 

3 - My wife was in the USA, and left it. Now she is outside of the usa. There was a question: "was beneficiary ever visited USA", I answered "yes". Do I need to upload I-94 document to proof she left the country? Will they send us RFE to show us proof that she left the USA?

Answer yes, beneficiary was in US. My husband has only visited the US and was not here at the time of submittal, so for I-94 record we put “not applicable”.  In that section we only filled in his passport number, country of issuance, and expiration. Travel document number we also put “not applicable”. Our petition was approved without RFE
side note: we did submit our I-94 record, but only in the evidence section as proof of visits to see me in the US.

Sounds like this is more than Mary Smith, becoming Mary Jones after marrying Jim Jones.  If so, provide evidence of the formal name change.  If she only took a "married name" or change from maiden to married name, the marriage certificate is all that's needed.  Ashley Brown, does not become Susan White, by simply marrying Mr. White.

No need to submit I-94 document as proof that your wife left the US.  That said, I recommend that you do submit her I-94 travel history (arrival/departure records) as part of your bona fide marriage evidence.  We submitted beneficiary's I-94 travel history.  As supporting evidence, we attached photos corresponding to every US visit in the I-94 history.  Our I-130 petition was approved, no RFE.
