A little confused about this question.

I lived at Address A from 2006-2012 and then for 4 months in 2016, shortly before moving to the US. This is also the address I used on all my US visa applications.
However, I lived at Address B from Mar 2015-May 2016.

Should I be using Address A or Address B for my last physical address?

Address B obviously. That's where you lived last.

I'm not sure I have this right. You were at address A, then address B for a short time, and then address A again?  If so, then address A is the last place you lived before coming to the United States, and that's the address you would use.

Were you living at address A the day you boarded a flight to the U.S.?

I was at address A when I boarded a flight to the US. Address A is where my parents still live.

Thank you, I apologize for the original post not being clear enough. 

So I take it doesn’t matter if my last stay at Address A was only for four months? And I should be okay with listing the dates as May-August, 2016?

I looked at the I-485 form, current edition.

 

I believe the question OP is trying to answer is:

 

"Provide your most recent address outside the United States where you lived for more than one year (if not already listed above)."

 

I still believe this would be address B. Because this is the last address where OP lived for over one year. Address A was the last address where OP lived but only for 4 months. Neither address A or B would be listed in question about addresses in the last 5 years since the lookback would be 2018.
