Good catch. I erroneously thought B visas were immune from the canada / mexico / nearby islands rule since https://travel.state.gov/content/travel/en/us-visas/tourism-visit/visitor.html does not discuss it.

 

So granny will have to go to costa rica to try to reset.

One thing I forgot to mention, OK I am not familiar with the Macedonian Insurance market, but getting travel insurance for an 81 year old for a long visit is likely to be tricky, getting travel insurance for a short stay is likely to be expensive.
