my fiance' and I are soon to get married here in florida. As a k1 holder, I only have my vaccination supplement copy from st. luke. Should I undergo another medical exam. here in the US for my AOS? IF YES, is it better if I have it before I apply for an AOS? and is the I-693 form is the same as VACCINATION DOCUMENTATION WORKSHEET that a k1 holder got from St. Luke?

any opinions are much appreciated!

The doctor at the K-1 medical normally hands over a DS 3025, which lists all of the vaccinations you've had. This needs to be copied over onto a Appendix A Supplemental Form to the I-693 (which VJ calls the I-693a) by a civil surgeon here in the USA.

You can submit your AOS without it but you may get RFEd for it at some point. If you are interviewed you will need definitely need to supply it then.

RFE=request for further evidence

It's one you receive a mail from immigration asking you additional documents (that you may or may not have already sent), an RFE often means a slow down in the process of your case as they'll only resume working on it when the files are complete.
