Yes, she needs to go through a medical exam. You can choose to either complete it and mail it in with I-485 or send it in at some point later after filing I-485.

I file form I130 for my wife who came to the state on visit visa ,she did not leave the state after her arrival,the I130 was routed to california service center,during the time of filing,i did not submit i485,I864,I765 and I131,we live in texas and i am trying to mail the 4 forms out but dont know if i should mail all to dallas lockbox ,i read that if you live in texas ,you have to mail the I485 to dallas lockbox,my question is that,

Should i mail all four pforms to the same lockbox?please thank you as i await your reply

u sent adjustment of status package in 1 envelope i-485, 765, 131 ( with i-130 receipt notice) to :

 

 

USCIS Dallas Lockbox

 

U.S. Postal Service (USPS):

USCIS
Attn: AOS
P.O. Box 650288
Dallas, TX 75265-0288

 

OR 

 

FedEx, UPS, and DHL deliveries:

USCIS
Attn: AOS (Box 650288)
