Knightsbridge Medical Questionnaire

Hi all, I have a question about medicals at Knightsbridge.

 

I've seen the medical questionnaire form IV18 they ask you to fill in before you go (https://uk.usembassy.gov/wp-content/uploads/sites/16/2017/05/USA-MEDICAL-QUESTIONNAIRE_IV.pdf), and I've seen the USCIS form I-693 that Knightsbridge fills in and the applicant has to sign (https://www.uscis.gov/sites/default/files/document/forms/i-693.pdf), and I've read the technical instructions for Civil Surgeons (https://www.cdc.gov/immigrantrefugeehealth/civil-surgeons.html).

 

I've read on other threads (e.g. https://www.visajourney.com/forums/topic/720502-knightsbridge-visa-medicals-experience-wednesday-the-2nd-of-october and https://www.visajourney.com/forums/topic/663623-knightsbridge-medical-visit-experience) that Knightsbridge asks you to fill in a longer medical questionnaire when you're there.

 

Does anyone have a copy of the longer questionnaire, or can you remember what questions it asks?

From memory, its more of an extended version of the questionnaire you have attached. 

 

Asks about certain diseases or hospital visits. It's nothing too invasive!

I don’t remember the questions, but it’s just more detailed about each thing that’s on the main questionnaire. 
 

Do you have a specific thing you’re concerned about? If so let us know what it is and we can see if we recall what questions are asked about it.

As above, I don't really remember but it's just more in depth than the one you do in advance. Also asks about some other stuff i.e. you have to list dates and details of all hospitalisations.

I just completed my medical on the 4/08/23 and I can confirm the second form you have to fill out is almost as identical to the first you pre filled out. Do not worry yourself, it's very straight forward with nothing on there to catch you out!

I did mine last week there. It's just a series of yes or no questions. Questions like have you ever had an operation, every had any problems with your liver, your kidneys, your bladder, if you've ever had any heart attacks etc. About 25 Qs like that that i suspect almost everyone will answer no to the vast majority (i was no to all). Literally took 1 min to tick all the boxes
